Surrender Questionnaire
This form is to be filled out if you are looking to surrender a dog to Crest Care so that we can assess if the dog is a candidate to come into rescue and if we have an appropriate open foster home.   Please complete all of the questions.  You will also need to contact your veterinarian and give them permission to speak with a Crest Care Representative and release records to us if needed. Please email photos of the dog(s) to crestcareinc@gmail.com. Your request will be addressed within 48 hours.
